Jacob Ptak, age 80, a resident of Columbiana, died on Tuesday, Nov. 1, 2011. Memorial services will be 3 p.m., Sunday, Nov. 6, 2011 at Columbiana Church of the Nazarene with Rev. Gary Tankersley officiating. Visitation will be from 1-3 p.m. prior to the service at the church. He served in the Army and was a painter for 47 years.

Mr. Ptak is survived by his wife, Shirely Bailey Ptak; step-children, James Bailey (Jane), Vonda Smith (Ray), Stanley Bailey (Connie), Joseph Bailey (Tammie), Saconda Vanderslice and Marty Bailey (Brandy); 13 grandchildren; 19 great-grandchildren; sister, Edith Navarre; and a host of nieces, nephews, family and friends.
