The following are uniform incident and offense reports from the Shelby County Sheriff’s Office from Oct. 13-19:

Oct. 13

-Domestic violence-harassment from the 100 block of Christy Drive, Shelby.

-Miscellaneous incident-property damage from the 57000 block of Alabama 25, Dunnavant. A 2013 Toyota Camry was damaged.

-Miscellaneous incident from the 3200 block of Shelby County 39, Chelsea.

-Miscellaneous incident from the 12000 block of Kenley Way, Birmingham.

-Theft of property fourth degree from the 4000 block of Shelby County 71, Shelby. Various toiletries and grocery items were stolen.

-Domestic investigation from the 11000 block of Kenley Way, Birmingham.

-Domestic violence-criminal surveillance, domestic violence-harassment from the 0 block of Rich Drive, Chelsea.

-Theft of property second degree from the 1500 block of Shelby County 336, Chelsea. A Smith & Wesson M&P 9-milimeter and Citalopram were stolen.

-Unlawful breaking and entering a vehicle from the 100 block of Sweet Gum Lane, Chelsea. U.S. currency in the amount of $50 was stolen.

-Theft of property (two counts) from the 100 block of Sweet Gum Lane, Chelsea. A 2013 Dodge Ram 1500 valued at $25,000, a 2009 Ford Escape valued at $10,000 and additional items valued at $7,971 were stolen.

-Theft of lost property from the 16000 block of U.S. 280, Chelsea. A wallet, $320 in cash, Social Security cards and Medicade card were stolen.

-Fire investigation from the 200 block of Country Drive, Columbiana. A house was destroyed by fire.

-Leaving the scene of an accident from U.S. 280 at Brookhighland Parkway, Birmingham. A 2017 Chevy Cruze was damaged.

Oct. 14

-Miscellaneous incident-property damage from U.S. 280 and Shelby County 442, Westover. A 2004 Ford Taurus SE was damaged.

-Theft of property fourth degree from the 4600 block of U.S. 280, Birmingham. An unknown amount of food items was stolen.

-Unlawful breaking and entering a vehicle from an unnamed location. A prescription for Oxycontin (15 mg) was stolen.

-Miscellaneous incident from Alabama 119 at Souther Boulevard, Birmingham. A curb was damaged.

-Miscellaneous incident from the 800 block of Shelby County 89, Montevallo.

-Domestic investigation from the 1000 block of Morning Sun Drive, Birmingham.

-Leaving the scene of an accident, property damage from the 2000 block of Stevens Creek Road, Birmingham. A 2012 Kia Optima was damaged.

-False identity to obstruct justice, possession of drug paraphernalia from the 100 block of Sassy Lane, Chelsea. A glass pipe with a bulb on the end containing drug residue was recovered.

-Assault from the 200 block of Clear Spring Way, Columbiana.

-Missing person-juvenile runaway from the 1000 block of Springfield Drive, Chelsea.

-Unlawful breaking and entering a vehicle, theft of property second degree from the 500 block of Tara Drive, Columbiana. A debit card, credit card, Walmart brand purse valued at $30 and a Social Security card were stolen.

-Burglary, assault from the 6300 block of Farley Lane, Birmingham. An LG Style 5 and a pair of reading glasses were damaged; and several pairs of snips, tubing cutters and hammers; Milwaukee drill set and Ryobi Sawzall and drill set were stolen.

-Possession of a forged instrument, theft of property from the 4600 block of U.S. 280, Birmingham.

-Fraudulent use of a debit or credit card from the 200 block of Lime Creek Lane, Chelsea. Unauthorized charges totaling $3,000 were made to the victim’s debit card.

-Theft of property first degree from the 300 block of El Camino Real, Chelsea. A Generac Guardian home generator valued at $8,000 was stolen.

-Domestic violence from the 4600 block of Chesapeake Cove, Birmingham.

-Miscellaneous incident-property damage from the 500 block of Shelby County 260, Maylene. A 2011 Ford Focus was damaged.

Oct. 15

-Miscellaneous incident from the 2200 block of Brock Circle, Birmingham.

-Duty to give information-hit and run from the 58000 block of Alabama 25, Leeds. A 2017 Toyota Corolla was damaged.

-Theft of property fourth degree from the 4900 block of Cahaba Beach Way, Birmingham. A metal trailer shutter valued at $10 was stolen.

-Harassment, following too closely from Shelby County 11 and Old Highway 280, Chelsea.

-Miscellaneous incident-hit a deer/property damage from the 5200 block of South Shades Crest Road, Helena. A 2019 Toyota Camry was damaged.

-Domestic violence third degree-harassment from the 600 block of Mountainview Drive, Wilsonville.

-Domestic investigation from the 100 block of Heatherbrooke Road, Birmingham.

-Obstructing justice using false identity from the 1400 block of West Navajo Drive, Alabaster.

-Domestic violence-assault from the 400 block of Stonecrest Drive, Birmingham.

Oct. 16

-Domestic violence-assault from the 1800 block of Shelby County 30, Columbiana.

-Domestic investigation from the 1400 block of Mimosa Road, Leeds.

-Harassing communications from the 100 block of Wild Ivey Lane, Maylene.

-Disturbing private image with intent to harass, threaten, coerce or intimidate from the 1000 block of Crawford Court, Chelsea.

-Unlawful breaking and entering a vehicle from the 3000 block of Chelsea Ridge Trail, Chelsea. A Remington 700 .308 rifle valued at $500, H&R .30-06 single shot rifle valued at $1,000, H&R .223 single shot rifle valued at $350, Thompson Center Encore .50-caliber muzzle load rifle valued at $1,620, 7-milimeter Remington Barrel and two Conoco hard molded gun cases valued at $220 were stolen.

-Theft of property from the 1700 block of Pedal Fall Drive, Birmingham. A 2017 Suzuki GSXR 1000 valued at $13,000 was stolen.

-Unlawful breaking and entering a vehicle from a parking lot outside 8000 building of Kenley Way, Birmingham. The victim’s checkbook was stolen.

-Receiving stolen property first degree from the 5300 block of U.S. 280, Birmingham. A Remington 700 rifle, 308 Winchester; New England Firearms Handirifle, 223 Remington; New England Firearms Handirifle, 30-06; Thompson Encore black powder rifle, 209×50 Magnum; and a Thompson Encore black powder rifle, 7-milimeter, barrel only was stolen.

-Death investigation from the 100 block of Branch Drive, Chelsea.

-Miscellaneous incident from the 500 block of Meadow Ridge Circle, Birmingham.

-Missing person-runaway juvenile from the 400 block of Kennebrew Drive, Chelsea.

Oct. 17

-Unlawful possession of marijuana, unlawful possession of drug paraphernalia from the 1000 block of Fairbank Lane, Chelsea. A green leafy substance believed to be marijuana (4 grams), a large glass pipe, small glass pipe and a grinder with the same leafy substance believed to be marijuana was confiscated.

-Burglary from the 8300 block of Shelby County 47, Shelby. A Sanyo flat screen TV valued at $500, LG flat screen TV valued at $300 and a PlayStation 4 valued at $450 was stolen.

-Identity theft from an unnamed location. Amounts of $950, $1,783 and $4,800 were stolen from checks.

-Miscellaneous incident from near the 30-mile marker of U.S. 280, Harpersville. A 2017 Ford F150 was damaged.

-Criminal mischief from the 7000 block of Meadowlark Drive, Birmingham. Large panes of glass valued at $1,200 and a glass door valued at $300 were damaged.

-Theft of property, burglary from the 12000 block of U.S. 280, Chelsea. An AED Make-Life Pak valued at $2,500 was stolen.

-Fraudulent use of a credit or debit card from the 16000 block of U.S. 280, Chelsea.

-Allowing animals to run at large from the 2000 block of Shelby County 45, Sterrett.

-Miscellaneous incident from the 100 block of Lakeview Circle, Vandiver. A Glock 9-milimeter pistol valued at $600 was recovered.

-Miscellaneous incident from the 1900 block of Island Road, Shelby.

-Domestic violence-harassing communications from the 5400 block of U.S. 280, Birmingham.

Oct. 18

-Harassment from the 8000 building of Kenley Way, Birmingham.

-Miscellaneous incident from the 5100 block of Prairie Drive, Birmingham.

-Miscellaneous incident from the 2100 block of Chelsea Park Bend, Chelsea. A small plastic baggy containing 1.05 grams of methamphetamine was confiscated.

-Miscellaneous incident from U.S. 280 West at Klein Road, Harpersville. A 2012 Mazda 3 was damaged.

-Miscellaneous incident from U.S. 280 West at Klein Road, Harpersville. A 2016 Jeep Patriot was damaged.

-Civil dispute from Somerby Assisted Living, 200 One Nineteen Boulevard, Hoover. A check in the amount of $6,249 was written on the victim’s State Farm account.

-Miscellaneous incident from the 3000 block of Brookhill Drive, Birmingham.

-Assault from Alabama 70 near Shelby County 26.

-Unlawful possession of a controlled substance from Alabama 25 and Shelby County 61, Wilsonville. A crystal white substance believed to be methamphetamine was confiscated.

-Miscellaneous incident from the 4200 block of Boulder Lake Circle, Vestavia.

-Missing person from the 100 block of Perkins Landing Road, Shelby.

-Identity theft from the 200 block of Shelby County 331, Columbiana.

-Miscellaneous incident from the Shelby County Jail.

-Death investigation from the 100 block of Chestnut Lane, Helena.

-Domestic violence-harassment, domestic violence-criminal mischief from Oak Mountain High School, 5476 Caldwell Mill Road, Birmingham. A Honda Civic sustained $200 in damages.

-Menacing from the 100 block of Hillside Lane, Sterrett.

-Identity theft from the 2300 block of Valleydale Road, Birmingham.

Oct. 19

-Missing person-juvenile from the 200 block of King’s Home Drive, Chelsea.

-Domestic investigation from the 2000 block of Eagle Ridge Drive, Birmingham.

-Promoting prison contraband from the 300 block of McDow Road, Columbiana. White pills believed to be Gabapentin (0.6 grams), several plastic Ziplock bags and half of a red straw were recovered.

-Possession of a forged instrument from the 70 block of Hollybrook Road, Columbiana. Counterfeit U.S. currency totaling $640 was recovered.

-Harassment from the 400 block of Kinnebrew Drive, Chelsea.

-Theft of property from the 900 block of Gorman Park Road, Vincent. A sign for Gorman Park Boat Launch was damaged.

-Receiving stolen property from the 1000 block of Argo Road, Columbiana. A Gorman Park sign was confiscated.

-Possession of marijuana first degree, possession of drug paraphernalia from the 100 block of Hollybrook Road, Columbiana. A green leafy substance believed to be marijuana (9.3 grams) was recovered.

-Domestic violence third degree-harassment from the 5300 block of Meadow Brook Road, Birmingham.

-Theft of property fourth degree from the 16000 block of U.S. 280, Chelsea. Property valued at $340.87 was stolen.

-Domestic violence-harassment from the 5700 block of Hunt Cliff Road, Birmingham.
